Welcome to Ember Medical. This website, embermedicalgroup.com (the “Site”), is operated by Ember Medical Group, LLC (“Ember Medical,” “we,” “us,” or “our”).

By accessing or using this Site, scheduling an appointment, purchasing a service or treatment package, or enrolling in an Ember Medical membership, you agree to these Terms & Conditions and any additional policies, consents, membership agreements, or other agreements applicable to the services you receive.

Medical Services

Ember Medical provides medical, wellness, and aesthetic services through appropriately licensed healthcare professionals.

Information presented on this Site is for general educational and informational purposes only. Website content is not individualized medical advice and is not intended to replace evaluation by a qualified healthcare professional.

Using this Site, submitting a contact form, scheduling an appointment, or participating in a discovery call does not by itself establish a provider-patient relationship. A provider-patient relationship is established only after appropriate clinical requirements have been satisfied and an Ember Medical clinician has agreed to provide care.

Eligibility for Treatment

Scheduling or purchasing a service does not guarantee medical eligibility for treatment.

Medical and aesthetic treatments remain subject to appropriate clinical evaluation, medical history, contraindication screening, informed consent, and provider judgment.

Ember Medical may postpone, modify, or decline treatment when a provider determines that doing so is clinically appropriate or necessary for patient safety.

Treatment Results

Individual results vary. No medical, wellness, or aesthetic treatment can guarantee a particular result.

Photographs, testimonials, educational materials, treatment descriptions, and examples of outcomes are provided for informational purposes and do not constitute a guarantee or warranty regarding an individual result.

Appointments

Patients are responsible for providing accurate contact information and arriving on time for scheduled appointments. Cancellations, rescheduling, late arrivals, and missed appointments are governed by the Refund & Cancellation Policy below.

Purchases and Treatment Packages

Certain services and treatment packages may be purchased through this Site or through Ember Medical’s scheduling and payment systems.

Purchasing a service or package does not override medical eligibility requirements. Treatment remains subject to provider evaluation and clinical appropriateness.

Unless otherwise stated in writing, treatment packages are intended for the original purchaser and may not be transferred to another individual.

Any package expiration date, included treatments, limitations, and other package-specific terms will be disclosed at purchase.

Memberships and Recurring Payments
Ember Medical offers recurring medical and aesthetic memberships. By enrolling in a membership, you authorize Ember Medical or its designated payment processor to charge the payment method on file according to the billing frequency and amount disclosed in the applicable membership agreement. Membership benefits, fees, billing terms, minimum commitments, renewal terms, and cancellation requirements are governed by the applicable signed membership agreement and the Refund & Cancellation Policy below.

If there is a conflict between this general website policy and a signed membership agreement, the terms of the signed membership agreement will control to the extent permitted by law.
Membership benefits have no cash value and are nontransferable unless expressly stated otherwise.

Prescription Medications

Prescription medications are provided only when ordered by an appropriately licensed healthcare professional following an appropriate clinical evaluation.

Nothing displayed on this Site constitutes an offer or guarantee to prescribe any particular medication or treatment.

Prescribing decisions, medication availability, pharmacy fulfillment, pricing, and insurance coverage may vary.

Payments
You agree to provide current, complete, and accurate payment and billing information for purchases and recurring charges.

Ember Medical may use third-party payment processors to securely process transactions.

Applicable pricing will be disclosed before purchase or enrollment.

This Site may contain links to third-party websites, pharmacies, financing providers, skincare retailers, laboratories, scheduling platforms, or other independent services.

Refund & Cancellation Policy

At Ember Medical, appointment times are reserved specifically for each patient. This policy explains our terms regarding appointment cancellations, late arrivals, treatment purchases, memberships, payments, and refunds.

Appointment Cancellations & No-Shows

We require at least 24 hours’ notice to cancel, reschedule, or make changes to any appointment.

Appointments cancelled or rescheduled with less than 24 hours’ notice, as well as missed appointments, are subject to a $100 cancellation fee, regardless of appointment type.

The applicable fee may be charged to the payment method on file.

Cancellation fees are charged for the time reserved and are not charges for medical treatment.

We understand that emergencies and exceptional circumstances may occur. Ember Medical Group, LLC may waive a cancellation fee at its discretion, and any exceptions required by applicable law will be honored.

Late Arrivals

Please arrive on time for your scheduled appointment. We allow a grace period of up to 10 minutes for unforeseen delays.

If you arrive more than 10 minutes late, your appointment may need to be shortened.

If you arrive 15 minutes or more late, or if there is insufficient time remaining to safely and appropriately complete the scheduled service, your appointment may need to be rescheduled and the $100 cancellation fee may apply.

Clinical quality and patient safety will not be compromised to accommodate a late arrival.

Weather

Colorado weather can change quickly. We encourage patients to review anticipated conditions before their appointment and make scheduling changes within the required 24-hour cancellation period whenever possible.

If Ember Medical closes the office or cancels an appointment because of weather or another circumstance, no cancellation fee will be charged.

Treatment Purchases

Payment for a service does not guarantee medical eligibility for treatment.

All medical and aesthetic services remain subject to provider evaluation, contraindication screening, and clinical judgment.

Completed treatments and services are nonrefundable.

Because individual outcomes vary, dissatisfaction with a treatment outcome does not, by itself, qualify a patient for a refund.

Treatment Packages

Unless otherwise stated in writing, treatment packages are intended for use by the original purchaser and are nontransferable.

Any package expiration date and other package-specific terms will be disclosed at purchase.

Completed treatment sessions are nonrefundable.

If Ember Medical determines that a patient is no longer medically eligible for an unperformed treatment, Ember Medical will review the circumstances and determine the appropriate resolution consistent with the applicable purchase terms and applicable law.

Membership Billing & Cancellation

Ember Medical offers different membership programs. Billing, cancellation, and refund terms vary by membership and are disclosed before enrollment.

Ember Aesthetics Membership

The Ember Aesthetics Membership has an initial six-month fixed term and is billed monthly.

The membership may not be canceled during the initial six-month fixed term except as permitted by the membership agreement or required by applicable law.

Unless properly canceled, the membership automatically converts to month-to-month status after the initial six-month term.

After completion of the initial fixed term, the membership may be canceled with at least 30 days’ written notice emailed to hello@embermedicalgroup.com.

Unused monthly treatments or membership benefits do not roll over, have no cash value, and are forfeited at the end of the applicable membership period.

Memberships are nontransferable and are not eligible to be frozen or suspended.

Additional rescission or cancellation rights may apply as provided in the signed Ember Aesthetics Membership Agreement or applicable law.

Ember Medical Membership — Monthly

Monthly Ember Medical Memberships are month-to-month.

Membership may be canceled at any time by providing at least 30 days’ written notice to hello@embermedicalgroup.com.

Membership remains active during the 30-day notice period.

Unused membership benefits do not carry forward and are not refundable.

Ember Medical Membership — Annual Prepaid

Annual prepaid Ember Medical Memberships remain active for 12 consecutive months.

If an annual prepaid membership is canceled before completion of the 12-month term, Ember Medical will calculate any applicable refund by valuing each month that has elapsed, including any partially elapsed month, at the then-current standard month-to-month membership rate and subtracting that amount, together with any outstanding balance, from the amount prepaid.

The annual prepayment discount does not apply to a partial membership term. If the recalculated amount equals or exceeds the amount prepaid, no refund is due.

At the end of the 12-month prepaid term, the membership automatically continues on a month-to-month basis at the then-current standard monthly rate unless another annual term is selected.

Once month-to-month, the membership may be canceled with at least 30 days’ written notice.

Payment Authorization

When a patient or member provides a payment method and authorizes Ember Medical to maintain it on file, Ember Medical or its designated third-party payment processor may charge that payment method for amounts the patient has authorized or agreed to pay, including scheduled services and purchases, membership dues, properly assessed cancellation or no-show fees, outstanding patient balances, and other purchases initiated or authorized by the patient.

Recurring membership charges are processed according to the billing schedule disclosed in the applicable membership agreement.

Failed or Declined Payments

If an authorized payment is declined or returned, the patient remains responsible for the outstanding balance.

Returned or failed payments may be subject to a fee disclosed in the applicable agreement and permitted by law.

Services or membership benefits may be suspended when an account has a past-due balance, subject to applicable law and applicable clinical obligations regarding continuity of care.

Accounts that remain unpaid may be referred for collection as permitted by law.

Refunds

Completed medical and aesthetic services are nonrefundable.

Properly assessed cancellation and no-show fees are nonrefundable.

Membership fees and refunds are governed by the applicable membership terms described above and the patient’s signed membership agreement.

Unused monthly membership benefits do not have cash value and do not roll over unless expressly stated otherwise.

Approved refunds will generally be returned to the original payment method.

Nothing in this policy limits any refund or cancellation right that cannot legally be waived.
